Declaration of Attendance at an Educational Institution
Dependent children of deceased members who are age 18 or more but less than age 25 must use this form to confirm their full-time attendance at an educational institution.
Declaration of Marital Status
Use to declare your current marital status and spouse or common-law partner, and former relationships, if applicable. The information is needed before your pension benefit can be processed and will be used to determine your survivor upon your death.
Canada/Quebec Pension Plan (C/QPP) Declaration Form
With this form, members who retire and are less than age 65 declare to the Pension Centre whether or not they are in receipt of C/QPP disability benefits at the time they apply for their pension benefit from Canada Post.
Transfer of pension benefits out of the Canada Post pension plan
For members and survivors who are entitled to a transfer of their pension benefit out of the Plan. Both forms are needed to finalize the transfer.
